How a Suit Should Fit in the Shoulder: Seam, Pad, and Divot
The shoulder seam should sit on the acromion. Pads can add 0–1.5 cm. A divot in the sleeve cap is a size or last problem. If the shoulder is wrong, walk away.

Find the bone, then watch the sleeve head
Feel for the acromion at the end of the collarbone ridge. The jacket’s shoulder seam should land there. A 1 cm extension can be a roped British shoulder; 2–3 cm of pad hanging in air is a size too big. If the seam sits inside the bone, the jacket is small — you will get tightness across the deltoid and a wrinkle that looks like a frown at the sleeve head. Raise your arms as if reaching for a laptop on a shelf. Pain or a lock-up means the scye (armhole) is too low or too tight.
This is the check that the first-suit guide refuses to skip. Sleeves and waist can move. Shoulders mostly cannot, not at alteration prices that make sense.
Pad, rope, and unstructured are different garments
Padded, canvassed: a defined line, 1–1.5 cm of pad, sometimes a slight rope (a raised sleeve-head ridge). Natural / spalla camicia: little pad, sleeve gathered into the head, softer. Unstructured: cloth, lining optional, almost no pad — a sport coat more than a boardroom suit. Mixing a huge pad with a skinny 14 cm trouser hem is two decades on one body. Pick a shoulder language and keep the rest of the suit in it.

Neck, collar, and the rest that follows
Jacket collar should hug the shirt collar with 1–1.5 cm of shirt showing at the back, no halo. A gaping collar is often a shoulder or upper-back problem, not a “collar roll” you press out. Chest: you should button the top button of a two-button without an X wrinkle. If the shoulder is right and the chest pulls, that is a size or a cut. Do not size up in the shoulder to fix a chest — you will swim. Try another maker. A 36–40 mm watch should pass under the cuff without dragging the sleeve head.
Questions, answered
- Can a tailor narrow the shoulders?
- Sometimes, a little, at a price. In the $400–800 band it is usually cheaper to buy the jacket that already fits the bone. Never buy a shoulder that is too tight hoping it will stretch.
- Is a roped shoulder old-fashioned?
- It is a British/American business language. A soft Italian shoulder is another language. Both work. A 3 cm power pad from 1989 does not.
- Why do I get dimples at the sleeve even in my size?
- The last does not match your shoulder slope. You may need a more sloped or a squarer shoulder, not a different numbered size.
